Ambulatory support tailored to you

Support can be essential when you are dealing with mental health challenges, behavioral problems or difficulties with thinking. Do you experience “moderate to severe limitations” because of a disability or impairment? Then Zorgplus Solutions offers support tailored to you, in the form of help, practice or supervision with your daily activities.

What do we support you with?

Together with you, we look for ways to increase your independence and organize your daily life better. For example:

  • building and keeping social contacts
  • planning daily activities
  • help with getting around, in and outside your home
  • organizing your household
  • support with parenting questions
  • learning to manage money and keep your paperwork in order
  • learning to live with a disability
  • encouraging you to take part in social activities
  • support in tackling an addiction (alcohol, drugs)

How do you apply for support?

For support you need an assessment (indicatie: the official decision on which care you are entitled to) under the Wmo (the Dutch Social Support Act, arranged through your municipality) or the Wlz (Long-term Care Act). Don’t have an assessment yet? Zorgplus Solutions will inform you and help you apply. You can also contact the neighborhood team (wijkteam) in your municipality yourself. As soon as you have an assessment, our team is ready for you.

Costs and coverage

The cost of support is covered through the Wmo. You do pay a personal contribution, depending on your income. The CAK (the government agency that handles these contributions) calculates this amount, and you pay it directly to the CAK.

Support in your own language

Our team speaks Dutch, English and Portuguese. So you can work on your goals with confidence, in your own language.
